Job Description:
A company is looking for an RN Case Manager for Home Health.
Key Responsibilities
Integrate health care for clients from pre-admission to
post-discharge through coordination of care
Evaluate outcomes based on clinical, financial, and utilization
data
Monitor and trend patient responses utilizing structured care
methodologies
Required Qualifications
Bachelor's degree in nursing preferred
Graduate from a nationally accredited nursing program preferred
Minimum of one to two years of professional nursing experience in a
clinic care setting preferred
Currently holds an unencumbered RN license with the State Board of
Nursing
Valid driver's license may be required depending on location
